THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

I. Who We Are

This Notice describes the privacy practices of OpenLoop Health, Inc. and its affiliated medical groups in connection with telehealth services facilitated through the Trinity Meds website.

II. Our Privacy Obligations

We are required by law to maintain the privacy of your PHI and to notify you of a breach within 60 days of discovery.

III. How We May Use and Disclose Your Health Information

  • We may use and disclose your PHI to provide treatment, diagnose and treat your illness, and share with other healthcare providers involved in your care.
  • We may use and disclose your PHI to obtain payment for services provided to you.
  • We may use and disclose your PHI for internal administration, quality improvement, and evaluation of healthcare professionals. We may also use AI/ML tools for these purposes in compliance with HIPAA.
  • We may disclose PHI to family members, relatives, or close friends when you agree, do not object, or when we reasonably infer you would not object.
  • We may disclose PHI for public health activities including disease control, child abuse reporting, FDA reporting, and communicable disease alerts.
  • We may disclose PHI in judicial or administrative proceedings and to law enforcement as required by law.

IV. Uses Requiring Your Authorization

For uses not described in this Notice, we require your written authorization, including for marketing and sale of PHI.

Special protections apply to mental health, substance abuse, HIV/AIDS, STDs, genetic testing, child abuse, domestic abuse, and sexual assault information.

V. Your Rights

  • You have the right to request restrictions on how we use and disclose your PHI for treatment, payment, and healthcare operations.
  • You may request access to your medical record and billing records. Submit a Release of Information Form to care@trinitymeds.com.
  • You have the right to request that we amend PHI in your records. Submit an Amendment Request Form to care@trinitymeds.com.
